NEW NAME, SAME COMMITMENT Celebrating 30 years providing comprehensive care to our Northern Colorado community.Started by an all-volunteer grassroots initiative in 1978, Hospice of Larimer County has recently expanded to serve Western Weld County. To reflect our growing service area and our community heritage, we have changed our name to Pathways Hospice, Community Care for Northern Colorado. The new name reflects our belief that each person’s end-of-life journey or grief process is unique. It also builds on our established, nationally recognized PATHWAYS Programs for Grief & Loss.
